In clinical practice, methanol poisoning results from ingestion, absorption through the stomach wall, inhalation, and skin absorption. Ingestion is usually accidental, and is the most common route of exposure to people who consume or manufacture methanol.Exposure can occur when workers are exposed to unintentional inhalation of aerosolized methanol, or are accidentally exposed to high concentrations of vapor. Skin exposure is usually due to over-dilution of professional solvents (paint thinner, lacquers, and glues), and exposure from ingestion of methanol-containing beverages such as salad drippings, or drinking from a car radiator drain. Second-degree classification means that methanol is moderately toxic. If ingested, it typically produces nausea, vomiting, abdominal cramps, and bloody diarrhea, often in combination with headaches. Severe methanol poisoning can lead to metabolic acidosis and a lack of oxygen to extremities, including the kidney. Methanol is generally toxic to the central nervous system and can lead to blindness or even death when administered in large doses. Inhalation of an abnormally high concentration is also fatal in most cases.
Methanol toxicity is due to damage to the central nervous system, especially the brain. Methanol, which is absorbed from the GI tract and enters the blood, is metabolized to formaldehyde in the liver, a compound that gets into all body cells and causes DNA damage. From a toxicological standpoint, methanol is a strong polar molecule, and therefore bypasses the blood-brain barrier. Both the brain and the other central nervous system cells are exposed to methanol and the metabolite formaldehyde.
